Lung hyperinflation in COPD: the impact of pharmacotherapy
نویسندگان
چکیده
Improvement in airway function in response to bronchodilator therapy is generally confirmed by simple spirometry. However, improvements in maximal expiratory flow rates have been shown to correlate poorly with important patient-centred outcomes, such as reduced exertional dyspnoea and improved exercise performance. Recent studies have suggested that attendant reductions in end-expiratory lung volume as a result of bronchodilator-induced improvements in lung emptying may be more closely associated with symptom relief and increased exercise capacity than traditional spirometric indices. To the extent that chronic lung hyperinflation and the superimposition of acute dynamic hyperinflation (in response to increased ventilation or expiratory flow limitation) result in excessive loading and weakening of the inspiratory muscles, then pharmacological lung volume reduction should have important mechanical and sensory benefits for the patient. The present article will examine the mechanisms of lung deflation following short-term bronchodilator therapy. The physiological links between reduced hyperinflation, improved dyspnoea and exercise endurance will be examined, and the emerging evidence for the additive effects of combining various modern pharmacological therapies will be reviewed.

The Evidence of Hyperinflation on Chest X Ray and its Correlation with Air Flow Obstruction in COPD Patients
Introduction: According to the latest statistical and epidemiological studies, COPD will become the fourth leading cause of death in 2030 worldwide. Scientists are studying on methods to diagnose COPD in the patients in early stages, because it is a curable and preventable disease in early stages. In this study, evidences of hyperinflation on ...
متن کاملThe impact of chronic obstructive pulmonary disease (COPD) and its associated lung hyperinflation on cardiac structure and function
Background Significant cardiovascular morbidity and mortality exists in COPD, independent of traditional risk factors, although the mechanisms are poorly understood. Lung hyperinflation, the presence of supra-normal residual volumes (RVol) at the end of expiration, is one postulated mechanism. We sought to investigate the impact of COPD on cardiac structure and function compared to a control gr...
متن کاملThe role of bronchodilator treatment in the prevention of exacerbations of COPD
Exacerbations of chronic obstructive pulmonary disease (COPD) are natural events in the progression of the disease, and are characterised by acute worsening of symptoms, especially dyspnoea. These heterogeneous events follow increased airway inflammation, often due to infection, and lead to decreased airflow and increased lung hyperinflation relative to stable COPD. Although exacerbation freque...
متن کاملCPAP decreases lung hyperinflation in patients with stable COPD.
BACKGROUND Dynamic hyperinflation, caused by expiratory flow limitation, markedly increases resting end-expiratory lung volume (functional residual capacity) in many COPD patients. OBJECTIVE To determine the impact and duration of impact of CPAP on hyperinflation and airway resistance in patients with stable COPD. METHODS In a case series, 21 patients underwent CPAP at 8 cm H(2)O for 15 min...
متن کامل[Exercise tolerance and dyspnea in patients with chronic obstructive pulmonary disease].
BACKGROUND/AIM Peripheral muscle weakness and nutritional disorders, firstly loss of body weight, are common findings in patients with chronic obstructive pulmonary disease (COPD). The aim of this study was to analyse the impact of pulmonary function parameters, nutritional status and state of peripheral skeletal muscles on exercise tolerance and development of dyspnea in COPD patients. METHO...
